This service develops care plans with clients that include strategies and goals to assist the person to remain living at home for as long as possible. It can include nursing services, occupational therapists, home modifications and gardening services. Commonwealth Respite and Carelink Centres

Commonwealth Respite and Carelink Centres provide information on local aged care and support services and link carers to appropriate services. Eligible carers can arrange short term or emergency respite through the centres
